This film highlights three innovative schemes in NHS South of Tyne and Wear. In Gateshead we are reshaping models of care and new ways of working with local GPs. In South Tyneside we have worked successfully in partnership with the local council specifically around health improvement. Finally in Sunderland there are ground-breaking schemes such as Urgent Care Teams based within Primary Care Centres.
