Improving Lives; Strength in Numbers

The film demonstrates how we go about targeting those most at risk from developing long-term health problems and/or supporting those who are most vulnerable and therefore likely to develop associated risks because they are disenfranchised from the health care system. The film also shows how primary care is used to bring about health improvement and how the PCT goes about targeting groups that do not use mainstream primary care centres.
